Mental Health Case Management

All across Hastings and Prince Edward County, our Mental Health Case Management Staff are able to meet with you in your home, community and/or one of our five office locations to assist you with your mental health recovery as you define it.  Your worker will help you to navigate the system, set goals and to get/stay connected to services that are important to you as you work towards wellness.  Support and advocacy are considered key components when assisting you to reach your goals and meet your needs.

Hospital Liaison Case Management

Before it is even time to leave the hospital, the Hospital Liaison will meet with you to help plan for your transition back into your community.  This worker’s objective is to assist you with making that process as seamless as possible in the location of your choice.  Following discharge, you’ll receive ongoing support and advocacy as you’re connected with services that are beneficial to your overall health.

Activities of Daily Living Peer Mentor

Grocery shopping, budgeting, household chores, recreation and access to community resources are just a few of the activities with which our ADL Peer Mentors can assist you.  Offering and delivering services that are guided by their own personal experiences, staff members from this program create a valuable connection with you as they support you on your journey to wellness.

Emergency Room Diversion Case Management

Whether it was an addiction and/or mental health concern that brought you to your local ER Department on a number of occasions, this transitional program offers a variety of services that are essential to connecting you with community supports and services aimed at meeting your recovery goals and needs, as well as reducing your use of the ER.

Transitional Case Management

This program focuses on providing short-term, transitional support to individuals our staff have recognized as requiring a rapid response to address immediate needs.  Focusing on your goals and needs, this worker will help you identify and connect with community supports and services that promote stability and well-being.

Skills Groups

Providing helpful information and practical techniques on a variety of topics, skills groups are offered in our various satellite offices across Hastings and Prince Edward County.  All of our groups provide an excellent opportunity to learn and to connect with staff and peers.

Concurrent Disorders Case Management

Providing integrated case management services to individuals dealing with both mental health and addictions concerns in the Belleville and Trenton area, this program emphasizes wellness and relapse prevention.  Concurrent Disorders Case Managers will assist with treatment planning, service navigation, support and advocacy among other things.
